DOCUMENTS REQUIRED FOR OBTAINING ISI CERTIFIATION IN Chutia:

  • Copy of acknowledgement expected from portal/ Screen shot of the online file submission
  • Address Proof (1) Business Licence               (2) Other Document issued by Govt.
  • Demand Draft like photocopy of DD or NEFT detail
  • If application is not signed by the CEO, Authority letter from CEO (Authorized signatory to be an employee of the company)
  • FORM VI Online generated copy (duly signed and sealed)

STEPS INVOLVED IN OBTAINING ISI CERTIFICATION IN Chutia

  1. Check whether or not your product falls below compulsory registration list
  2. Submit Product for psychiatry at BIS qualified lab
  3. Pay progress online or through a dd
  4. Submit online application in Form V and get sticking together of a copy of such form after affluent submission
  5. Submit everything the documents in hard copy along gone duly signed Form V past the department
  6. Department shall conduct a site visit
  7. Issuance of ISI Certificate.
