Consent to Operate:

Once Consent to Establish is issued by the department, the Unit shall apply for Consent to Operate in order to carry on its operations. CTO shall be issued solitary after a prior inspection and visit of the unit. CTO shall be issued by the department within a mature of 30 days.

2. Copy of CTE or previous CTO
