Consent to Operate:
Once Consent to Establish is issued by the department, the Unit shall apply for Consent to Operate in order to carry upon its operations. CTO shall be issued on your own after a prior inspection and visit of the unit. CTO shall be issued by the department within a period of 30 days.

- Copy of CTE or previous CTO
- Copy of Environmental Compliance Report
