• Copy of acknowledgement traditional from portal/ Screen shot of the online file submission
• Address Proof (1) Business Licence               (2) Other Document issued by Govt.
• Demand Draft once photocopy of DD or NEFT detail
• If application is not signed by the CEO, Authority letter from CEO (Authorized signatory to be an employee of the company)
• FORM VI Online generated copy (duly signed and sealed)

2. Submit Product for psychiatry at BIS recognized lab
3. Pay progress online or through a dd
4. Submit online application in Form V and buy a copy of such form after well-to-do submission
5. Submit whatever the documents in difficult copy along subsequent to duly signed Form V as soon as the department
6. Department shall conduct a site visit
7. Issuance of ISI Certificate.
