DOCUMENTS REQUIRED FOR OBTAINING ISI CERTIFIATION IN Kolkata:

• Copy of acknowledgement acknowledged from portal/ Screen shot of the online file submission
• Address Proof (1) Business Licence               (2) Other Document issued by Govt.
• Demand Draft afterward photocopy of DD or NEFT detail
• If application is not signed by the CEO, Authority letter from CEO (Authorized signatory to be an employee of the company)
• FORM VI Online generated copy (duly signed and sealed)
• Undertaking

2. b) For IS 13252 (Part 1) amendment 2, if applicable
3. c) For IS 16046 Changeover to 2015 version, if applicable

STEPS INVOLVED IN OBTAINING ISI CERTIFICATION IN Kolkata

1. Check whether or not your product falls under compulsory registration list
2. Submit Product for psychiatry at BIS recognized lab
3. Pay go ahead online or through a dd
4. Submit online application in Form V and buy a copy of such form after booming submission
5. Submit whatever the documents in hard copy along in the same way as duly signed Form V taking into account the department
6. Department shall conduct a site visit
7. Issuance of ISI Certificate.
